Do I have to transfer to Dental Care Ireland?

No, should you not wish for Dental Care Ireland to continue your dental care, you have the right to transfer to an alternative dental practitioner of your choice. For this we ask that you complete the “Consent for Release of Information” form and forward it to us at info@fortfielddental.ie or return it to the practice in person prior to the 1st October 2023. We at Fortfield Dental will then transfer your data (in the form of your patient records) your choice of dentist in a secure manner. You can also request a copy of your notes for your own records and we can organise secure delivery. If you have any queries at all in regards to the above, please call the practice on (01) 492 7288 or email us on info@fortfielddental.ie.

Do I need to re-register with the practice once ownership transfers to Dental Care Ireland?

  • No, you will not need to do anything; the change of ownership of the practice has no effect on your registration or your entitlement.

What happens to my data if I choose to do nothing?

  • If you do nothing, your patient records will transfer to Dental Care Ireland. You have the option at any time to transfer to another dental practice, at which time your file will be forwarded to the new practice, and your patient chart will be placed in an inactive status by Dental Care Ireland.

We at Fortfield Dental will assist you in any way possible throughout this transition. Should you have any queries, please contact us on either (01) 492 7288 or email us on info@fortfielddental.ie.

We appreciate the continued support and loyalty of our patients and look forward to welcoming you and your family to Dental Care Ireland for many years to come.
